1. Availability - Call our Community Relations Manager (518) 576-9210 to ask about availability.
2. Medical Overview – Provide KVNH with an overview of the potential resident’s medical history, current ailments, and prognoses.
3. Determining Rates - The resident needs to advise their monthly income to determine the affordability of KVNH’s standard rates. If their income is not high enough, a discounted rate may be available. In this case, the resident should provide more details about their income and available assets, and KVNH will determine whether a discount can be offered. Please note this is not always possible. A financial worksheet is available to help with calculations. Please note that a one-time admission fee is also charged. This fee varies depending on your resources. VETERANS - if you are a veteran who has served during wartime years, you may qualify for additional income benefits through the VA which helps pay for your care. Please feel free to ask us more about this benefit.
4. In-Person Evaluation – A representative of KVNH will visit the potential resident where they are currently located (hospital, nursing home, their own home, etc.) to verify their physical and mental abilities. This is a simple process and takes approximately 10-15 minutes. A video meeting can also be arranged if necessary.
5. Application – Fill out the resident application.
6. Negotiated Rate Letter – Once the resident has agreed to the negotiated rate for care, a signed, Negotiated Rate Letter will be issued.
7. Payment – The resident should provide payment for the non-refundable admission fee at this point. The monthly long-term care rate will be collected on move-in day.
8. Permissions - The resident (or the resident’s Healthcare Proxy/POA) should advise the primary care doctor and/or current healthcare professional that a bed is available at KVNH and that KVNH has permission to speak with them about the resident’s medical history.
9. TB TEST – A TB test needs to be given to the resident. If the resident has already received a TB test within 30 days of the move-in date, the test results can be forwarded to us. If time is of the essence, a chest x-ray can be used (“no active TB noted” must be written on the x-ray).
10. Medical Orders and Medications – If the resident is in the hospital, a nursing home, or a rehabilitation center, the attending doctor(s) must provide a signed medical evaluation form and provide orders for care, including any referrals needed for continued care after discharge. KVNH will send a copy of the medical evaluation form to the doctor. New prescriptions for all medications, including any over-the-counter creams, ointments, or supplements, must be sent to our pharmacy. The resident cannot be discharged into our care until all this paperwork has been completed and reviewed by KVNH.
1. Arrange a move-in date – Moving in typically takes place Monday-Thursday before 1:00 pm. Because we need to review and coordinate any medications required by the resident, any discrepancies that may arise can be rectified on weekdays, but they cannot be rectified on weekends, which could put the Resident at risk. However, weekend admissions may be possible if certain conditions can be met and with the approval of the Administrator.
2. Sign the resident agreement – The contract/agreement for residency at KVNH will be signed on the move-in date. The first month’s room and board fee will be due at this time.
3. Personalized care plan – Our RN or LPN will sit down with the Resident to review their medical needs, current prescriptions, dietary needs and preferences, hobbies, etc.
4. Accommodations – KVNH will supply the Resident’s room with a bed, pillow, linens, bath towels, nightstand, dresser, chair, floor lamp, bedside lamp, clock, and window dressings. Residents moving in for long-term care can bring whatever items they like from home, including furnishings. However, we recommend that you do not bring items of value or family heirlooms. KVNH is not responsible for any items that go missing or get damaged.
5. TV & phone service - If you would like a TV or phone in the room, this needs to be supplied by you. Cable TV and landline phone service are available at an additional charge and are solely the responsibility of the Resident. Free Wi-Fi is available in most rooms, so a Roku or Firestick device, or a Smart TV with apps may be a low-cost option to consider.
6. Clothing – Bring at least 7 days’ worth of seasonal clothing, including shoes and coats. All clothing MUST be labeled with the Resident’s name. Personal hygiene items, such as Depends underwear, are solely the responsibility of the Resident. We do not recommend bringing any expensive items.
